Crossroads Treatment Centers is seeking a Care Coordinator to meet face-to-face with patients, facilitate recovery services, and ensure proper documentation in the patient records. This role supports the delivery of evidence-based MAT options and coordinated care across the treatment team.

Qualifikationen

• Bachelor’s degree in a related field and at least one year of experience with substance use disorders or co-occurring mental illness.
• RN licensure with at least one year of direct experience serving people with SUD or co-occurring disorders.
• CSAC certification or CSAC-Assistant under supervision as defined by 18 VAC 115-50-10.

Aufgaben

• Meet face-to-face with patients to facilitate recovery services and document in the patient’s record.
• Utilize collateral contacts and local referral agencies to provide services not offered by the program.
• Conduct monthly Care Coordination meetings with patients and document to ensure engagement with needed services.
• Collaborate with the buprenorphine-waivered practitioner prescribing buprenorphine or naltrexone for individuals with moderate to severe OUD.
• Perform other duties as assigned by the Operations Director of OBOT Services.
