Job summary

Crossroads Treatment Centers is seeking a Care Coordinator to meet with patients in person, ensuring recovery services are properly documented and coordinated with collateral contacts and local referrals. You will support monthly care coordination meetings and work with the buprenorphine- or naltrexone-prescribing clinician to facilitate MAT for individuals with opioid use disorder.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Psychiatric Rehabilitation, Sociology, Counseling, Vocational Rehabilitation, or Human Services Counseling with 1+ year experience in substance use disorders or cooccurring conditions.
• Registered Nurse license with 1+ year experience with substance use disorders or cooccurring mental illness.
• CSAC or CSAC-Assistant certification under supervision as defined in 18 VAC 115-50-10.

Responsibilities

• Meet face-to-face with patients to assist in recovery services documentation.
• Utilize collateral contacts and local referral agencies for services not offered by the program.
• Conduct monthly Care Coordination meetings and document patient engagement.
• Collaborate with buprenorphine‑waivered practitioners for medication-assisted treatment administration.
• Perform other duties as assigned by the OBOT Operations Director.
