Job summary

Crossroads Treatment Centers is seeking a Care Coordinator in Knoxville to support patients with opioid use disorder. You will meet face-to-face with patients, document care, and coordinate services with the treatment team.

Work with buprenorphine/naltrexone prescribers, utilize local referrals, and lead monthly care coordination meetings. A bachelor’s degree and RN or CSAC certification are preferred for this role.

Qualifications

• Bachelor's degree in Social Work, Psychology, or related field required.
• RN license with experience in substance use or cooccurring disorders preferred.
• CSAC certification or CSAC-Assistant option required or as defined.

Responsibilities

• Meet face-to-face with patients to facilitate recovery services and document in the patient record.
• Utilize collateral contacts and local referral agencies to provide services not offered by the program.
• Conduct monthly Care Coordination meetings and document outcomes to ensure engagement.
• Collaborate with the buprenorphine/naltrexone prescriber for individuals with OUD.
