Job summary

Crossroads is seeking a Care Coordinator to meet with patients, document recovery activities, and coordinate services with collateral contacts and referral partners. The role collaborates with the buprenorphine/naltrexone prescriber to support individuals with opioid use disorder.

The position requires a bachelor’s degree in a relevant field and licensure or certification (RN or CSAC) with experience in substance use disorders, and involves Monday–Friday hours with various supportive benefits.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Psychiatric Rehabilitation, Sociology, Counseling, Vocational Rehabilitation or Human Services Counseling.
• License as a Registered Nurse with relevant experience in substance use disorders.
• Board of Counseling CSAC or CSAC-Assistant certification under supervision.

Responsibilities

• Meet face-to-face with patients to facilitate recovery services and document in the patient record.
• Use collateral contacts and local referrals to provide missing services.
• Lead monthly Care Coordination meetings to ensure patient engagement.
• Collaborate with buprenorphine- or naltrexone-prescribing practitioner for OUD care.
• Perform other duties as assigned by OBOT Operations Director.
