Job summary

Crossroads Treatment Centers is seeking a Care Coordinator to meet with patients, coordinate recovery services, and document care plans. You will work with collateral contacts and local referrals to provide needed services, and lead monthly care coordination meetings to keep patients engaged in their treatment journey.

The role collaborates with a buprenorphine/naltrexone prescriber for opioid use disorder and supports the OBOT program under the Operations Director; benefits include professional

Qualifications

• A bachelor’s degree in social work, Psychology, Psychiatric Rehabilitation, Sociology, Counseling, Vocational Rehabilitation, or Human Services Counseling and at least one year of experience with substance use disorders or cooccurring mental illness.
• RN licensure with at least one year of direct experience serving individuals with substance use disorders or cooccurring diagnoses.
• CSAC or CSAC-Assistant under supervision as defined in 18 VAC 115-50-10.

Responsibilities

• Meet face-to-face with patients to facilitate recovery services and document in the patient record.
• Use collateral contacts and local referral agencies to provide services not offered by the program.
• Conduct monthly Care Coordination meetings and complete documentation to ensure patient engagement.
• Collaborate with the buprenorphine-waived practitioner treating individuals with opioid use disorder.
• Perform other duties as assigned by the Operations Director of OBOT Services.
