Job summary

Crossroads Treatment Centers seeks a Care Coordinator who will meet face-to-face with patients to facilitate recovery services and ensure accurate documentation in the patient record, supporting continuity of care.

You will collaborate with the buprenorphine-waivered practitioner, conduct monthly Care Coordination meetings, and perform duties as assigned by the OBOT Operations Director across outpatient clinics nationwide.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Psychiatric Rehabilitation, Sociology, Counseling, Vocational Rehabilitation, or Human Services Counseling and 1+ year experience with substance use disorders or cooccurring mental illness.

Responsibilities

• Meet face-to-face with patients to facilitate recovery services and document care.
• Coordinate with collateral contacts and referral agencies to provide services.
• Conduct monthly Care Coordination meetings and ensure engagement with needed services.
• Collaborate with buprenorphine-waivered practitioner for MAT.
• Perform other duties as assigned by the OBOT Services Operations Director.
