Job summary

Crossroads Treatment Centers is seeking a Care Coordinator to meet with patients, document recovery activities, and coordinate services across local referral networks. You will work closely with a buprenorphine-waivered practitioner to support individuals with opioid use disorder in a collaborative clinical environment.

The role emphasizes scheduling, documentation, and engagement in recovery activities, with opportunities for professional growth in a supportive outpatient setting.

Qualifications

• Bachelor’s degree required in relevant field (see education section).
• Licensed/credentialed as CSAC or CSAC-Assistant under supervision (18 VAC 115-50-10) or RN with relevant experience.
• Experience providing services to individuals with substance use disorders or cooccurring mental illness is required.

Responsibilities

• Meet face-to-face with patients to facilitate recovery services and document in their records.
• Coordinate with collateral contacts and referral agencies to provide services not offered by the program.
• Conduct monthly care coordination meetings and ensure patient engagement with required services.
• Collaborate with buprenorphine-waivered practitioner prescribing buprenorphine or naltrexone.
