Job summary

Crossroads Treatment Centers in the United States seeks a Care Coordinator to support patients through recovery services, document care, and coordinate with local referral partners. You will work closely with buprenorphine or naltrexone prescribers to ensure integrated MAT care and accurate patient records.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Psychiatric Rehabilitation, Sociology, Counseling, Vocational Rehabilitation, or Human Services Counseling.
• Registered Nurse licensure with at least one year of direct experience serving individuals with substance use disorders or cooccurring mental illness.
• Board of Counseling Certified Substance Abuse Counselor (CSAC) or CSAC-Assistant under supervision as defined in 18 VAC 115-50-10.

Responsibilities

• Meet face-to-face with patients to facilitate recovery services and document in the patient’s record.
• Utilize collateral contacts and local referral agencies to provide services not offered by the program.
• Conduct monthly Care Coordination meetings with patients and complete documentation to ensure engagement with needed services.
• Collaborate with buprenorphine- or naltrexone-prescribing practitioner for individuals with moderate to severe OUD.
• Perform other duties as assigned by the Operations Director of OBOT Services.
