Job summary

Arkansas Provider Coalition is seeking a remote Care Coordinator to support Summit Community Care, serving Union County and surrounding areas. You will develop PCSPs, coordinate services, and maintain ongoing contact with members, families, and providers to ensure timely, person-centered care.

The role requires a bachelor’s degree in a social science or health field and at least one year of Excel/Outlook/Word experience, plus strong communication and HIPAA compliance.

Qualifications

• One year of beginner-level experience with Microsoft Excel, Outlook, and Word.
• Bachelor’s degree in social work, psychology, sociology, or a health-related field.
• Equivalent combinations of education and experience may be considered for the role.

Responsibilities

• Develop and maintain the annual Person-Centered Service Plan (PCSP) for assigned members.
• Maintain monthly contact with members, families, providers, and care team.
• Conduct quarterly in-person visits; frequent monthly visits for complex cases.
• Document work activities into the EMR within 24 hours and ensure HIPAA compliance.
