Job summary
Arkansas Provider Coalition, LLC is seeking a Care Coordinator I to support Summit Community Care members by developing and monitoring individualized PCSPs and coordinating behavioral and physical health services. You will maintain ongoing member contact, collaborate with families and providers, and document in the EMR within 24 hours.
This remote/mobile role requires a valid Arkansas driver's license and reliable transportation for visits; strong English communication is essential, with Spanish
Qualifications
- Bachelor’s degree in social work, psychology, sociology, or health-related field.
- One year or more of experience with individuals with developmental or behavioral disabilities and/or substance use disorders.
- Proficiency with MS Excel, Outlook, Word; ability to use databases and EMR systems.
- Knowledge of HIPAA and confidentiality requirements.
Responsibilities
- Develops, documents, evaluates, and monitors the annual PCSP for members.
- Coordinate behavioral health, physical health, and community resources.
- Maintain monthly contact with members, families, providers, and care team.
- Document activities in EMR within 24 hours; ensure regulatory compliance.
