The role involves updating support plans, coordinating meetings, and ensuring services run smoothly. Experience in DSP, caregiving, or case management is a plus and local travel may be required.

Responsibilities

• Coordinate customer and provider support with regular check-ins.
• Maintain accurate documentation in support notes and demographics.
• Respond to inquiries via calls, emails, texts, and voicemails.
• Coordinate meet-and-greets between customers and providers.
• Identify service gaps and communicate needs to the team.
• Help obtain approvals for protocols and action plans.
• Train providers on customer routines and documentation expectations.
• Escalate safety issues or service disruptions when needed.
• Complete accurate service documentation for DSP shifts.
