The Care Coordinator I works in an interdisciplinary team to assess client needs, conduct individual and group counseling, facilitate sessions, deliver curricula, and help clients navigate care systems while maintaining a supportive environment.

Qualifications

• High school diploma or equivalent required.
• Bachelor's degree in health and human services, social work, psychology, or related field preferred.

Responsibilities

• Facilitates individual counseling sessions with each caseload client.
• Facilitates group counseling sessions and community meetings; teaches classes on treatment topics.
• Documents client updates and incidents in the facility log daily via email.
