Job summary

HealthRIGHT 360 is seeking a Case Manager I to coordinate care across outpatient offices, residential programs, and community settings. You will assist with intake, apply for Medi-Cal, and connect clients to benefits and services as part of a multidisciplinary team.

The role requires reliable transportation and supports sensitive populations through a trauma-informed, evidence-based approach. Union-eligible, full-time, with a 8:00am-4:30pm schedule.

Qualifications

• Case management experience in human services.
• Experience with clients experiencing withdrawal from substances.
• Knowledge of substance use treatment and relapse prevention.

Responsibilities

• Assist with intake by completing case management assessment and entering financial and benefit information.
• Support the client in applying for Medi-Cal or transferring Medi-Cal County when appropriate.
• Coordinate communication with probation, DCFS, housing, and other services.
• Transport clients as needed.
• Complete client UA tests and document test results.

Substance Use Disorder Counselor registration (CCAPP/CAADE/CADTP)
