Job summary

The Harris Center for Mental Health and IDD is seeking a Care Coordinator to perform clinical assessments and develop recovery plans in collaboration with the treatment team. This community-based role requires travel to consumer homes or field locations and adherence to Trauma Informed and Patient-Centered principles.

Qualifications

• Bachelor’s degree in a social, behavioral, or human service field required.
• Minimum of one year experience in the mental health field preferred.
• Must maintain a valid Texas Driver's License and be insurable under Agency policy.
• Ideal candidate may be required to transport consumers in a personal vehicle; mileage reimbursed by the Agency.

Responsibilities

• Conduct comprehensive clinical assessments to determine service needs and track progress toward treatment goals.
• Collaborate with the treatment team to develop personalized recovery plans and obtain LPHA approval.
• Ensure services meet eligibility criteria and regulatory guidelines.
• Deliver skills training and psychosocial rehabilitation to address severe mental illness and related challenges.
• Provide crisis interventions to alleviate symptoms and prevent escalation.
• Assist consumers in accessing necessary care and services.
• Educate consumers on mental illness, medication importance, and related information.
• Monitor progress toward treatment goals and eligibility requirements.
• Meet productivity standards and maintain ethical conduct in all activities.
• Adhere to agency policies, procedures, and documentation requirements.
