HealthRIGHT 360 is seeking a Care Coordinator I for the African American Healing Center in San Francisco to support clients through assessment, counseling, and navigation of care services. The role emphasizes individual and group counseling within an interdisciplinary team.

Qualifications

• High school diploma or equivalent required; bachelor’s degree preferred.
• Must be registered as an AOD/SUD Counselor with CA certifying agency (CCAPP, CADTP, or CAADE).
• Current First Aid and CPR or ability to obtain within 30 days; CA driver’s license required.

Responsibilities

• Facilitates individual counseling sessions for each client.
• Leads group counseling sessions and community meetings; delivers curricula.
