Job summary

Renown Health seeks a Care Coordinator to coordinate all components of care across face-to-face, virtual, or telephonic visits, collaborating with clinical and non-clinical team members to create a longitudinal plan of care for patients with chronic needs.

You will identify social determinants of health, support transitions of care, document in Epic HealthPlanet and MIDAS, and align with CMSA standards; cross-training within the department may occur.

Qualifications

• English proficiency to perform job duties safely and effectively.
• RN licensure in the State of Nevada required.
• One year RN experience preferred; care management or home health experience preferred.

Responsibilities

• Coordinate patient care across disciplines to develop a patient-centered plan of care.
• Collaborate with patient, family, providers and team to support self-management goals.
• Document assessments and care plans in the Epic HealthPlanet module and MIDAS.
