Job summary

WellSense Health Plan is seeking a Clinical Care Manager to provide holistic care management for members, developing ICPs, and coordinating services across care settings. The role emphasizes self-management, psychosocial and socioeconomic supports, with a focus on improving outcomes and reducing costs.

The position supports telephonic outreach and in-person visits, uses EMR data, and collaborates with a multidisciplinary team to promote preventive care and adherence to treatment plans.

Qualifications

• Bachelor’s degree in nursing or Associate’s degree in Nursing with relevant work experience.
• 3 years of related experience in home health care or managed care organization.
• 3 years clinical experience with members who have multiple, chronic or complex health conditions.
• 2 years experience in care management, care coordination and/or discharge planning.

Responsibilities

• Completes targeted general and condition-specific assessments.
• Develops an Individual Care Plan with the member.
• Identifies barriers to self-management and coordinates care.
• Facilitates access to benefits and community resources.
• Demonstrates motivational interviewing to support health behavior change.
• Documents in the care management system in real-time during visits or calls.
• Coordinates care transitions and follow-ups with PCPs and specialists.
• Mentors staff and collaborates with multidisciplinary teams.
