Job summary

Ascend Medical is seeking a Remote Care Coordinator to deliver longitudinal Care Management services, including patient engagement, chart and care plan review, and collaboration with patients, families, providers, and interdisciplinary team members.

This fully remote role requires proactive outreach, accurate documentation, and the ability to identify barriers to care, coordinate follow-up, and meet productivity targets while maintaining HIPAA compliance.

Qualifications

• Healthcare background and care coordination experience.
• Experience in primary care or ambulatory settings preferred.
• Familiarity with care management documentation and payer requirements.
• Bilingual skills are a plus.

Responsibilities

• Deliver longitudinal care management, patient engagement, and care plan review.
• Coordinate with providers, families, and care team; document activities.
• Perform timely documentation and ensure HIPAA-compliant practices.
• Identify barriers to care and escalate as needed.
• Engage patients and caregivers to support adherence and outcomes.
