Job summary

Ennoble Care in Cincinnati, OH, is seeking a full-time Care Coordinator, Embedded, to join our patient-centered home-based care team. You will coordinate care with clinicians, staff and families, manage care plans in the EMR, and help patients achieve healthcare goals in a compassionate, organized way.

The role requires excellent communication, the ability to multitask, and proficiency with Microsoft Office.

Qualifications

• Must be comfortable with speaking on the phone for long periods.
• Compassionate and empathetic toward patients with exceptional customer service.
• Ability to take accurate notes and document tasks promptly.
• Multitask between patients and workstreams while staying organized.
• Thrives in a fast-paced environment and maintains professionalism.
• Proficient in using a computer, including Outlook and Microsoft Office programs.
• Knowledge of basic healthcare terms, conditions, roles and care principles.
• Must pass drug screen and background check; be a team player and adaptable.

Responsibilities

• Complete individualized patient care plans and perform care management and care coordination using EMR.
• Frequent contact with patients to provide coordination and ensure program compliance.
• Document all client communications accurately; maintain records.
• Coordinate with Primary Care Clinician or designee on emergent needs and care plans.
• Inform Team Supervisor of issues related to care plan processes.
• Collaborate across departments to meet patient and facility needs.
• Become proficient with technology including secure email and EMR systems.
• Maintain daily work logs and escalate client issues as needed.
• Provide excellent customer service and be a positive team member.
