Qualifications

• Must be comfortable speaking on the phone for large portions of the day.
• Compassionate and empathetic with exceptional customer service.
• Proficient in computer usage, including Outlook and Microsoft Office.

Responsibilities

• Develop individualized patient care plans using Ennoble Care's EMR.
• Maintain frequent contact with patients to coordinate care and ensure program adherence.
• Document all client communications accurately.
• Coordinate with Primary Care Clinician on emergent needs and care plans.
• Adhere to documentation protocols and log daily work activities.
