Job summary

Ennoble Care in Cleveland, OH seeks a full-time Care Coordinator, Embedded, to join the team at a senior living facility. You will work with clinicians, staff and families to plan and manage care, using our electronic medical record system.

This in-office role emphasizes compassionate service, scheduling support for caregivers, and clear communication to ensure high-quality care and compliant care plans. Daytime hours, Monday through Friday, with competitive hourly pay and benefits.

Qualifications

• Must be comfortable speaking on the phone for large parts of the day.
• Compassionate and empathetic toward patients with exceptional customer service.
• Ability to take accurate notes and document tasks promptly.
• Ability to multitask between patients and work streams while staying organized.

Responsibilities

• Complete individualized patient care plans and perform care coordination using Ennoble Care's EMR.
• Maintain frequent contact with patients to provide coordination and support to improve outcomes.
• Document all client communications accurately (verbal or written).
• Communicate with Primary Care Clinician or designee regarding patient needs and updates to care plans.
• Keep Team Supervisor informed of issues related to the care plan and patient needs.
• Collaborate with cross-organizational teams to meet patient and family needs.
