Job summary

Ennoble Care in Cincinnati, OH is seeking a full-time Patient Care Coordinator who shares our motto, “To Care Is An Honor.” You will work with clinicians, staff and families to ensure high-quality care and clear communication.

You will manage individualized care plans, perform care coordination in our EMR system, and maintain accurate documentation while keeping the care team informed of issues and patient needs.

Qualifications

• Must be comfortable speaking on the phone for long periods.
• Compassionate and empathetic towards patients with excellent customer service.
• Ability to take accurate notes and document tasks promptly.
• Ability to multitask between patients and workstreams while staying organized.

Responsibilities

• Complete individualized patient care plans and perform care management and care coordination using Ennoble Care’s EMR system.
• Frequent contact with patients to provide care coordination, support, and manage compliance with the care management programs to increase positive outcomes.
• Document all client communications accurately.
• Communicate to and from Primary Care Clinician regarding patient emergent needs and ensure care plans are complete and accurate.
• Keep Team Supervisor informed of all issues pertinent to the care plan process.
• Demonstrate ability to work with cross-organizational areas to meet patient needs and support families.
