Job summary

Ennoble Care is seeking a dedicated Care Coordinator, Embedded to support a senior living facility in Hamilton/Florence, NJ. You will coordinate care with clinicians, staff, and families, ensuring high-quality services and clear communication. Strong organization and compassionate patient care are essential.

You will manage care plans, document interactions, and use the EMR to track progress. This full-time role offers comprehensive benefits and a collaborative team environment.

Qualifications

• Must be comfortable with speaking on the phone for large portions of the day.
• Compassionate and empathetic toward patients with exceptional customer service.
• Ability to take accurate notes and document tasks timely.
• Ability to multitask between patients and workstreams while staying organized.
• Proficient in using a computer and Microsoft Office programs.

Responsibilities

• Create individualized patient care plans and perform care management using the EMR.
• Maintain frequent contact with patients to provide coordination and support for care programs.
• Document all client communications accurately.
• Communicate with Primary Care Clinician regarding patient needs and update care plans.
• Keep supervisor informed of issues related to care plans.
