Job summary

Centene is seeking a field-based Care Coordinator in Arkansas to support care management activities. You will engage members via phone or home visits, discuss care plans, coordinate with providers and caregivers, and help ensure continuity of care and satisfaction.

The role requires 1–2 years of related experience and a high school diploma or GED (or a relevant bachelor’s). Travel across the service area is expected.

Qualifications

• Requires High School diploma or GED.
• Requires 1–2 years of related experience.
• Bachelor's degree in social science/health-related field or HS diploma with 1 year care coordination experience (Arkansas).
• Must have driver's license and pass background/drug screening as applicable.

Responsibilities

• Perform member outreach by phone or home visits to discuss care plan and resources.
• Coordinate care activities with providers, partners, and caregivers.
• Handle member/provider inquiries related to care plans.
• Communicate with care managers and practitioners to facilitate services and continuity of care.
• Support service assessments/screenings and document member needs.
• Maintain member records per regulatory requirements and share as needed.
• Identify referral opportunities to Care Manager or Disease Manager.
• Educate members on benefits and available resources.
• Perform other duties as assigned.
• Adhere to policies and standards.
