Centene Corporation is seeking a full-time Care Manager support role in Kansas. You will engage members by phone or home visits, discuss care plans, and coordinate with healthcare partners to ensure seamless services and satisfactory outcomes.

Responsibilities include documenting plans, assisting with assessments, and guiding members through benefits and resources. Requires high school diploma or GED and 1–2 years of related experience; AR/FL content may apply, with travel up to 30% as needed.

Qualificações

• Requires a High School diploma or GED.
• 1–2 years of related experience.
• Bachelor's degree in social science/health-related field can substitute experience.
• Must reside in Kansas and be able to travel if required.
• Behavioral Health experience preferred.

Responsabilidades

• Provides outreach to members via phone or home visits to discuss care plans and education.
• Coordinates care activities with providers and partners to ensure continuity of care.
• Supports member and provider inquiries related to the care plan.
• Communicates with care managers and practitioners to enable services.
• Maintains member records and documents in compliance with regulations.
• Identifies needs and makes referrals to appropriate care resources.
• Performs other duties as assigned and adheres to policies.
