Qualifications

Responsibilities

• Outreach to members via phone or home visits to engage and discuss the care/service plan.
• Coordinate care activities with providers, partners, and members to address changes or progress.
• Respond to inquiries or concerns about the care/service plan and provide education on benefits.
• Document member records and ensure regulatory compliance.
