Responsibilities

• Outreach to members via phone or home visits to discuss care plans and next steps.
• Coordinate care activities based on the plan with providers and members/caregivers.
• Assist with inquiries or concerns related to care plans and services.
• Communicate with care managers and practitioners to ensure continuity of care.
• Support service assessments or screenings and document member care needs.
• Maintain member records in compliance with state and regulatory guidelines.
• Refer members to Care Managers or community resources as needed.
