Job summary

Centene Corporation is seeking a Care Coordinator II to provide non-clinical administrative support to the care management team in a dynamic health services environment. You will handle inbound/outbound calls, manage queues, collect eligibility information, and route referrals to clinical staff to support continuity of care.

You'll engage members by phone or visits where appropriate, document care plans, assist with assessments, and ensure compliance with regulatory standards while delivering

Qualifications

• Requires strong communication over the phone and via documentation.
• Experience handling care coordination workflow and referrals is a plus.
• Ability to follow regulatory guidelines and maintain member records.

Responsibilities

• Provide outreach to members by phone to discuss care plans and next steps.
• Coordinate care activities with providers and care managers.
• Handle member and provider inquiries related to care plans and services.
• Document member records and care needs in the system.
• Assist with service assessments or screenings when applicable.
• Maintain compliance with standards and policies.
