Cadence Care Network seeks an OhioRISE Care Coordinator to focus on family-based, responsive services using the High-Fidelity Wraparound model. This role involves developing Child and Family Teams, leading team meetings, and ensuring care plans conform to Ohio Department of Medicaid standards.
Candidates must have experience in children's behavioral health or related fields and meet educational and licensing requirements.

The OhioRISE Care Coordinator is responsible for cultivating flexible, family-focused, community-based responsive services based on the High-Fidelity Wraparound model of care coordination. Maintain fidelity to models of care coordination when conducting Intensive and/or Moderate Care Coordination activities. Develop and maintain the Child and Family Team, including coordinating and leading team meetings. Coordinate and supervise implementation of the Plan of Care, including a Transition Plan and Crisis Plan, through service delivery with providers and community resources; update the plan according to Ohio Department of Medicaid and agency requirements. Experience in children’s behavioral health, child welfare, developmental disabilities, juvenile justice, or a related sector human services or behavioral health care field, providing community-based services to children and youth, and their family or caregivers.
