Job summary

Enhabit Home Health & Hospice in Montgomery, AL is seeking a Scheduling Specialist to coordinate patient care with field clinicians and other care partners. You will manage assignments and ensure seamless communication to support quality outcomes.

Requirements include an LPN/LVN license, CPR, at least one year of clinical experience, and strong tech skills. In-office attendance is essential to supervise staff, coordinate shifts, and maintain documentation.

Qualifications

• High school diploma or equivalent is required.
• Licensed practical or vocational nurse with active license.
• CPR certification required.
• Minimum one year of clinical experience in healthcare.
• Valid state driver license and automobile liability insurance.
• Proficient with mobile devices; basic tech skills.
• Preferred bilingual English/Spanish to assist patients.

Responsibilities

• Schedule patients to field clinicians and coordinate care.
• Communicate with field staff, patients, physicians, and caregivers to ensure continuity of care.
• Maintain on-call notebook and hospitalization logs to improve communication.
