Job summary

Wellstar Health System is seeking a Senior Care Coordinator RN to assess transitional care needs and coordinate across the care continuum within a hospital setting. The role partners with physicians and care teams to drive efficient treatment plans and plan post-discharge care, while mentoring new hires and supporting care progression.

The RN Sr will lead interdisciplinary rounds, manage discharge plans, and ensure timely documentation and patient education to meet discharge goals.

Qualifications

• Associates degree in nursing or nursing diploma; bachelor preferred.
• RN licensure in Georgia with eNLC parity.
• Minimum 4 years as staff nurse in acute care and 2 years as hospital case manager.

Responsibilities

• Assess transitional care needs and coordinate across the continuum.
• Engage with patient and family to assure care needs are met.
• Attend patient/family care conferences, interdisciplinary rounds, and education.
• Mentor new hires and precept student interns.
• Participate in orientation and professional development; adhere to policies.
