Job summary

Qualifications

• 1 year nursing experience in the acute care setting.
• Excellent written and verbal communication skills.
• Knowledge of local, state and federal regulations and community resources.

Responsibilities

• Assessment of patients’ chronic disease management needs and psychosocial factors to support discharge planning.
• Manage discharge planning and ensure timely coordination with physicians, care teams, and payers.
• Collaborate in interdisciplinary rounds to optimize patient progression and transition to next level of care.
• Document assessments, plans, and actions in the medical record; track avoidable days and discharge barriers.
• Support professional development, onboarding, and adherence to Wellstar policies and standards.
