Job summary

Wellstar Health System in Georgia seeks a skilled Care Coordinator RN (CC RN) to assess transitional care needs, coordinate across the continuum, and engage patients and families to ensure discharge readiness and optimal outcomes.

You will plan care progression, collaborate with physicians, UR, and the care team to optimize resources and facilitate timely discharge, while documenting thoroughly and ensuring patient education.

Qualifications

• RN licensure required upon hire; state license preferred.
• Minimum 1 year nursing experience in acute care.
• Strong written and verbal communication; ability to work under pressure.
• Knowledge of healthcare regulations and community resources.
• Ability to collaborate with physicians, care team, and patient/family.

Responsibilities

• Assessment of chronic disease management needs and psychosocial risks.
• Discharge planning and coordination with physicians, PAS, and care team.
• Facilitate care progression, interdisciplinary rounds, and post-acute referrals.
• Document in medical records; track discharge milestones and avoidable days.
• Engage patient/family education and ensure timely transitions of care.
• Participate in professional development and adhere to policies.
