Job summary

Wellstar Health System is seeking a Care Coordinator RN (CC RN) to assess transitional care needs and coordinate care across the continuum. You will engage with patients and families to ensure needs are met, manage length of stay, and promote efficient resource use.

You will collaborate with physicians, utilization review, and the interdisciplinary team, coordinating discharge plans and post-acute referrals while addressing barriers and ensuring timely progression of care.

Qualifications

• RN licensure and active practice required.
• Minimum 1 year nursing experience in acute care setting.
• Excellent written and verbal communication skills.

Responsibilities

• Assess chronic disease management and psychosocial risk factors.
• Coordinate discharge planning and post-acute referrals.
• Collaborate with physicians, care team, and family to facilitate care progression.
• Document care plans and progress in medical records.
