Job summary

Cardinal Health is seeking a Patient Navigator in Fresno to address patient needs and coordinate with physicians and clinical staff. You will facilitate access to resources, collect data on barriers, and serve as the main contact for patients interacting with our care teams.

Ideal candidates have 0-2 years of experience and a Bachelor’s degree or equivalent, with preferred backgrounds in direct patient care or social work.

Qualifications

• 0-2 years of experience, preferred
• Bachelor's degree in related field, or equivalent work experience, preferred
• Candidates with direct patient care or Social Work experience, highly desired
• Ability to calmly and professionally resolve patient issues with diplomacy

Responsibilities

• Assess patient care needs for new and ongoing patients
• Coordinate and facilitate access to resources with physician and clinical office
• Serve as central contact between patients and providers across departments
• Collect and report data on patient barriers and intervene to improve care
• Assist patients with referrals to outside resources and care coordination
• Follow departmental policies and continuous quality improvement objectives
• Collaborate with providers and staff on patient and staff education
• Design or participate in creation of educational materials
• Perform miscellaneous duties as assigned
