Job summary

Cardinal Health is seeking a Breast Nurse Navigator in Fresno to coordinate patient care from initial imaging through the multispecialty cancer team. The role requires RN licensure in California, CPR, and strong patient-focused communication.

You will bridge primary care, imaging, and surgical teams, expediting diagnostic workups and ensuring timely appointments. The position emphasizes independent work, superb organization, and a patient-centered approach.

Qualifications

• Graduated from an accredited school of nursing with a diploma, Associate or Bachelor’s degree in Nursing.
• Eight years of RN experience, highly preferred.
• Current California Registered Nurse License
• CPR certification required.
• Strong customer service background, preferably in healthcare environment
• Excellent verbal communication skills
• Ability to manage and prioritize multiple tasks
• Proficiency in computer processing functions and other standard office equipment
• Ability to calmly and professionally resolve customer issues with diplomacy and tact.
• Ability to work independently with minimal supervision and flexibility with changing environments.
• Strong organizational skills.

Responsibilities

• Coordinate care from imaging center through the breast specialty clinic and cancer care team.
• Collaborate with radiologists during biopsy procedures, obtain consent, provide post-procedure care, and communicate results.
• Obtain pathology results and share findings with referring physicians and patients as appropriate.
• Serve as liaison between PCPs and cancer care team, coordinating timely appointments.
• Coordinate consultations with the multidisciplinary team and act as the single contact for breast cancer patients.
• Expedite workups for newly diagnosed patients and coordinate metastatic evaluations.
• Conduct post-operative follow-up calls to assess recovery and questions.
• Triage phone calls and direct patient needs to the appropriate team members.
• Review results with patients and schedule follow-ups under physician direction.
• Support neoadjuvant therapy patients and assist with follow-up and barrier resolution.
