Job summary

CrossWinds Counseling & Wellness is seeking a Patient Navigator to coordinate care and guide clients through intake and referrals. The role emphasizes trauma-informed interactions, strong data management, and collaborative teamwork to close referral gaps and ensure quality care.

The position requires excellent interpersonal and organizational skills, with responsibilities including intake interviews, chart openings, and ongoing coordination with clinical staff to support clients’ health goals.

Qualifications

• Bachelor’s Degree in Human Services or equivalent.
• Experience in case management or care coordination.
• Completion of Case Management Training within 6 months of hire.
• Proficiency with Microsoft Office and data systems.

Responsibilities

• Achieves expected referral rate and coordinates with internal/external providers to improve outcomes.
• Builds professional relationships with clients and families using trauma-informed, respectful approaches.
• Meets productivity and data collection requirements for CMHC/CCBHC programs.
• Communicates key client information to service providers before and after appointments; ensures next steps are clear.
• Maintains timely documentation reflecting services and client responses.
• Conducts initial in-person intake interviews and gathers demographic, financial, insurance, and health history data.
