Job summary

Crossroads Health is seeking a Patient Navigator-Behavioral Health to serve as the first point of contact for clients. The role focuses on trauma-informed intake, documentation, screening, and coordinating care for individuals and families seeking behavioral health services.

Responsibilities include supporting walk-ins and calls, coordinating with treatment teams, and maintaining clear communication to ensure timely access to care. OH licensing and QMHS readiness are part of the requirements.

Qualifications

• Must have an Associate’s Degree in Human Services or related field.
• Bachelor’s degree in social work or related field preferred.
• Licensed in Ohio with LPC/LSW/MFT preferred.
• Willing to acquire QMHS within 15 days of hire.
• Prior experience in medical office setting with substantial patient interaction preferred.

Responsibilities

• Provide trauma-informed services to clients primarily on the phone and in the office.
• Assist clients in completing initial required documentation.
• Provide trauma-informed screening, triage and care coordination to clients and their family system.
• Facilitate communication among treatment providers and with clients to ensure engagement and access to care.
