Qualifications

• Demonstrated healthcare experience in a clinic or hospital setting.
• Customer service focus with strong communications and problem resolution.
• Proficiency in advanced computer skills.
• Caregivers must maintain current BLS certification, have a valid driver’s license, auto insurance, and reliable transportation.

Responsibilities

• Acts as liaison between department and external organizations (payers, physicians, post-acute agencies, patients, families).
• Receives and prioritizes requests and transmits clinical information for service authorizations.
• Monitors referral status and maintains ongoing communication.
• Requests and retrieves medical records for retrospective utilization review or QA.
• Delivers routine regulatory notices within required timeframes with proper documentation.
• Performs clerical tasks to support care management services (reports, scheduling, communications).
• Supports advance care planning by delivering advance directive information and notarizing documents on request.
• Ensures provider lists are current and distributed prior to patient choice consultations.
• Assists in transition planning by preparing transfer packets and coordinating transportation.
• Collaborates with care managers to navigate community resources addressing social determinants of health.
• Works with clinical teams to ensure coordinated care and monitors patient care plans.
