Job summary

Mercy Medical Center Merced is seeking a Care Coordination Manager to provide operational leadership for the Care Coordination department, aligning with the organization’s care model under the Director of Care Coordination.

You will oversee care progression, discharge planning, patient advocacy, and collaboration with physicians and interdisciplinary teams to meet department priorities and improve outcomes.

Qualifications

• Bachelor of Science in Nursing (BSN) required upon hire.
• Master's Degree in Social Work required upon hire.
• License required for Masters Social Worker.
• Minimum of 3 years of clinical care coordination experience or 3 years of acute nursing leadership experience with knowledge of care coordination and discharge planning.
• Operational healthcare experience, implementation, and staff development.
• Certified Case Manager within 12 months or Accredited Case Manager within 12 months.

Responsibilities

• Provides operational leadership for the Care Coordination department through supervision, coaching, and performance management.
• Manages daily department operations including staffing, scheduling, productivity, and resource allocation.
• Ensures timely completion of patient assessments, discharge planning, utilization management collaboration, documentation and care transition activities.
• Monitors departmental performance metrics, identifies trends, and implements corrective actions.
• Leads multidisciplinary rounds and collaborates with physicians, nursing, Utilization Management and post-acute providers.
• Supports regulatory readiness and accreditation activities by maintaining compliance and documenting processes.
