Job summary

Mercy is seeking a Care Management Associate to support the interdisciplinary team with transition planning, length of stay management, and information coordination in Epic EMR systems. Qualifications

• Requires High School Diploma or equivalent.
• Experience in healthcare settings preferred.
• BLS (CPR) certification at start date or within 90 days.
• Strong communication and organizational abilities.

Responsibilities

• Communicates patient/family concerns with Care Managers and Social Workers to ensure ongoing needs are addressed.
• Provides assistance and support for the Care Management team.
• Deliveries and pickups for prescriptions at Mercy Tower Retail Pharmacy.
• Obtain transport quotes for patient/post-acute transitions.
• Distribute letters and facility lists to patients/families; support care plans.
