Job summary

St. Joseph’s Health is seeking a Care Management leader to oversee department operations and optimize patient outcomes through effective care coordination and discharge planning. The role emphasizes cross-functional communication and clinical guidance to staff across utilization management and care transitions.

You will partner with medical leadership to implement and evaluate an integrated Care Management program, ensuring staffing, training, and ongoing education align with systemic goals.

Qualifications

• Bachelor's Degree in Nursing or closely related field required.
• Master's degree preferred for advanced leadership/clinical practice.
• Three to five years of previous work related experience recommended.
• RN licensure by New Jersey or LSW licensure by New Jersey required; CCM/ACM certification preferred.

Responsibilities

• Manage department to ensure Care Management operations and workflow.
• Provide clear communication across the continuum to improve patient outcomes.
• Serve as a clinical resource and guide staff in utilization management, discharge planning, and care coordination.
• Collaborate with Care Management Leadership and Administration on program implementation, maintenance, and evaluation.

Education
