Job summary

The Department of Health and Social Services (DHSS) in Delaware is seeking a Health/Human Service Case Manager II to determine financial and technical eligibility for Medicaid State Plan programs and SNAP. This role manages a high-volume caseload and completes initial determinations, annual redeterminations, and changes while responding to inquiries from applicants, recipients, and providers.

Qualifications

• Six months' experience in case management including assessing, planning, developing, implementing, monitoring, and evaluating options and services.
• Six months' experience making recommendations as part of a client's service plan.
• Six months' experience interpreting laws, rules, regulations, standards, policies, and procedures.
• Six months' experience using automated information systems to enter, update, modify, delete, retrieve and report on data.
• Six months' experience in narrative report writing.

Responsibilities

• Determine financial and technical eligibility for Medicaid programs for Delaware residents.
• Maintain a high-volume caseload and complete initial determinations and redeterminations.
• Respond to inquiries from applicants, recipients, and stakeholders.
• Maintain case records and required forms.
• Coordinate services and connect clients with needed resources.
