Qualifications

• Six months' experience in case management including assessing, planning, developing, and evaluating options.
• Six months' experience making recommendations as part of a client's service plan.
• Six months' experience interpreting laws, rules, and procedures.
• Six months' experience using an automated information system to manage data.
• Six months' experience in narrative report writing.

Responsibilities

• Interviews clients, families, or professionals to determine needs for health and human services.
• Develops cost-effective service plans utilizing in-house or contracted providers.
• Monitors and evaluates service delivery for quality and efficiency.
• Maintains comprehensive case records and required reports.
• Establishes effective working relationships with clients and community sources.
• Provides information to connect clients with needed services and coordinates care.
• Communicates with clients on-site, via email or telephone.
• Uses information systems to research, review, and report on health/human service data.
