Job summary

Qualifications

• Six months' experience in case management including assessment, planning and monitoring.
• Six months' experience in making recommendations as part of a client's service plan.
• Six months' experience in interpreting laws, rules, regulations, standards, policies.
• Six months' experience in using an automated information system to enter, update, modify, delete, retrieve and report data.
• Six months' experience in narrative report writing.

Responsibilities

• Interview clients and sources to determine needs for health and human services, including employment and basic skills.
• Develop comprehensive service plans using in-house or contracted providers.
• Monitor and adjust service delivery for quality and efficiency.
• Maintain case records, summaries and required forms.
• Build effective relationships with clients, families and professionals.
• Provide information and referrals for services and coordinate on-site or via email/phone.
