Qualifications

• Can work EST or CST.

Responsibilities

• Coordinate administrative, operational, and customer support activities for care management programs and services.
• Support assessment, coordination, and facilitation of members' care needs to achieve health outcomes.
• Collaborate with members, providers, caregivers, and care teams to improve patient wellbeing and outcomes.
• Use discretion to prioritize requests and apply established procedures with independent judgment.
