Job summary

Carewell is seeking a remote Care Concierge to guide new members through enrollment, welcome visits, and ongoing care navigation. You will coordinate with primary care providers, pharmacies, and community resources to close gaps and improve member outcomes.

You will work in a flexible, pod-based environment with assignments driven by queue volume and staffing needs, and you will contribute to evolving processes as the program grows.

Qualifications

• 1+ years of member-facing healthcare experience or care coordination experience.
• Ability to work across enrollment, welcome visits, and care navigation.
• Authorized to work in the US without employer sponsorship.

Responsibilities

• Enroll prospective members, explain program benefits, and secure participation.
• Conduct welcome visits and set up ongoing coordination for members.
• Coordinate care gaps, engage providers, and connect community resources.
• Document interactions in real time and follow compliance gates.
• Collaborate with the pod to ensure seamless transitions and outcomes.
