Job summary

Curana Health is hiring a Care Ally, Case Manager to join the interdisciplinary care team. You will assess, plan, implement, coordinate, monitor, and evaluate services to meet members’ health and social needs, acting as a liaison among Members, RP/POAs, PCPs, and Align Senior Care stakeholders.

The role reports to the Supervisor of Case Management and requires autonomy in a hybrid setting with travel to facilities 2–3 days per week.

Qualifications

• One year of clinical practice in case management, home health, critical care, discharge planning, or related areas.
• Proficiency with Microsoft Office and ability to navigate multiple systems.
• Case management certification preferred.
• Long-term care or nursing facility experience preferred.
• Active RN or LPN license in the state where duties are performed; or Bachelor’s degree in social work (MSW preferred).

Responsibilities

• Execute strategies and goals set by leadership to improve overall member experience.
• Contact plan members to conduct health assessments and develop plans of care.
• Educate members and families about disease status, plan benefits, and available resources.
• Collaborate with interdisciplinary care team and medical directors to coordinate treatment.
• Follow up with members to assess progress toward goals.
• Coordinate care with the team for non-urgent health questions and care planning.
• Assist with scheduling appointments, home health coordination, and utilization of benefits.
