Job summary

Curana Health, Inc. seeks a Care Ally, Case Manager to join our interdisciplinary care team. This role involves coordinating care, educating members and families, and ensuring services align with care plans across facilities.

The ideal candidate holds an active RN or LPN license (state-specific) or a BS/WSW; case management experience is preferred. The position requires travel 2–3 days weekly and offers a hybrid work arrangement.

Qualifications

• At least 1 year of clinical practice experience in case management, home health, medical/surgical, discharge planning, or related fields.
• Proficiency using Microsoft Office (Word, Excel, Outlook) and ability to navigate multiple systems.
• Case management certification preferred; long-term care or nursing facility experience preferred.

Responsibilities

• Lead interdisciplinary care planning and care coordination for members.
• Educate members, families, and caregivers about disease status, benefits, and resources.
• Coordinate with care team and RP/POAs to implement and monitor care plans.
• Schedule and facilitate appointments and home health / therapy visits as needed.
• Communicate health updates to members and RP/POAs; document care team meetings.
