Job summary

Curana Health is seeking a Care Ally, Case Manager to join our interdisciplinary care team and coordinate care for members across facilities and home settings.

You will conduct comprehensive health assessments, develop care plans, educate members and families, and serve as a liaison among members, RP/POAs, and the care team. The role combines field visits with remote coordination in a hybrid schedule.

Qualifications

• One year of clinical practice in case management, home health, or related areas.
• Proficient in Microsoft Office and able to navigate multiple systems.
• Case management certification preferred.
• Long-term care or nursing facility experience preferred.
• Active RN or LPN license or BSW/MSW with licensure preferred.

Responsibilities

• Execute care strategies to improve member experience and outcomes.
• Contacts plan members to conduct health assessments and develop plans of care.
• Educate members and families about disease status, plan benefits, and resources.
• Coordinate with the care team and RP/POAs to facilitate treatment.
• Routinely follow up with members to monitor progress toward goals.
• Schedule appointments and assist with home health and equipment coordination.
• Document care team meetings and distribute care plans to stakeholders.
• Support care team meetings with facilities and ensure timely updates.
